At the Center for Regenerative & Aesthetic Wellness and the Center for Primary Care, your comfort, safety, and overall experience are our top priorities from the moment you walk through our doors.
During your first visit, we take the time to understand you. We’ll review your medical history, discuss any previous treatments, and talk through your goals so we can create a clear, personalized approach. Many of our services are fully customizable, allowing us to tailor each treatment to your unique needs.
From there, we’ll walk you through recommended treatments, services, or products designed to address your concerns and build a thoughtful treatment plan together. This is also your time to ask questions and share any concerns so you feel confident and informed every step of the way.

Before your first appointment, be sure to fill out all the necessary paperwork ahead of time. If you are unable to complete it beforehand or need assistance, please arrive at least 30 minutes before your appointment to complete your paperwork.

If you are unable to complete and print these forms prior to your visit, please arrive at least 30 minutes prior to your scheduled appointment to complete paperwork in the office. If you have extensive medical history to document, we recommend arriving more like 45 minutes early.
Please bring your picture ID, insurance card, and medication list (with dosages).
If you need to cancel your appointment, we ask that you do so 24 hours in advance. Failure to do so will result in a $50 no-show fee. Thank you in advance!
